Planning ahead to ensure your wishes are known and respected.
We encourage all patients to complete an advance health care directive, which is your voice and conveys your values regarding your stated preferences for medical treatments and to select a person to make your health care decisions in case you are unable to do so. As a Catholic healthcare ministry, we seek to honor your human dignity and understanding your wishes.

Please provided a copy of your advance health care directive to your primary care physician and bring a copy with you when you are scheduling a procedure or admitted to the hospital. Your advance health care directive will be placed in your medical chart.
