How Strong Futures Works
Strong Futures begins during an adolescent’s stay in St. Mary’s Adolescent Behavioral Unit. For patients who may benefit from additional support after discharge, the care team works closely with the adolescent and family to determine whether participation in Strong Futures is appropriate.
The program is designed to build on the progress made during hospitalization by providing continued support, guidance and care coordination during the transition home.
During Hospitalization
Adolescents and families work with the care team to:
- Identify strengths, goals and challenges
- Develop a personalized transition plan
- Build coping and communication skills
- Connect with community resources
- Prepare for a successful return to home, school and daily life
After Discharge
For eligible participants, Strong Futures may provide:
- Care coordination
- Therapy support
- Medication management support
- Family engagement and education
- Ongoing skill-building
- Connections to community resources
Support may continue for up to six months following discharge.
Family Involvement Matters
Strong Futures recognizes that recovery does not happen in isolation. Family participation is a key component of the program, with parents and caregivers actively involved throughout treatment and transition planning.
By strengthening communication, supporting healthy routines and reinforcing the skills adolescents develop during treatment, families play an important role in helping their child build confidence, maintain progress and successfully transition back to home, school and community life.
